Has the phone been dropped or gotten wet at all?
If not it's either a loose screen cable connection on the systemboard or a faulty screen.
See the Most Helpful Answer to a similar problem in the same phone as it states how to replace the screen in the phone. This will allow you to check the cable connection first and if it is OK then most likely you'll have to replace the screen
