Optical drive cannot insert or eject

I have model cuh-1215a and can't figure why my optical drive won't insert or eject. I've check all fuses on the motherboard and they're all good. I've tried the optical drive in another good ps4 with the same ribbon cables and it worked so I know it's not the drive. I've checked the ribbon cable slots and those look clean too. I can manually insert a game and it will load OK when I restart the console. Upon ejecting there is a beep.

I'm at a lost on what could be wrong or what I could further do. The only thing I haven't done is rebuild the database. Anyone have any insights?
