So I tried restarting both my controllers and console, even restore my console to factory settings, still my controllers won’t connect. Then I tried plugging my controllers in with a USB cable but even then they wouldn’t connect to my PS4, only worked in the safe mode. One guy told me the motherboard Bluetooth controller chip is damaged but I saw other posts about the Bluetooth antenna coming loose?? but if the antenna was the issue wouldn’t the USB work??

yes usb would work if the antenna was loose as its only for wireless connection, in safe mode try a database rebuild

True, the USB would still be operational if the Bluetooth chip was damaged or the antenna came loose on the Bluetooth chip. There is no reason why the USB would stop working with the Bluetooth chip unless the USB had been disconnected or faulty. It could be a Bluetooth antenna going loose or disconnected, But if it's affecting the USB also then there might be something wrong with the code or there may be something wrong with the connectors inside your PS4.
