The DualShock 4 CUH-ZCT2 was announced in 2016 to replace the CUH-ZCT1 controller released in 2013. This controller is similar to the previous model.

I can't find the model number cuz it got erased at the back after playing for 3 years.. Cuz I need to replace my battery

Hey best thing would be to take the controller apart, there is a model number on the battery itself. That way you can order the exact part that you may need.
