I put in the disk inside my ps4. It goes into the game fine then boom "Cannot continue using application. The disc cannot read. Make sure that the disc is not damaged or dirty. This has happened with every PS4 game I've tried. Ive tried restarting the ps4 but no help. And i dont want to delete everything so it could work. Please help!!! Also the background sound can still be heard in the blue screen where it says close app and remove disc.

This is usually happens for tow reson
1- over heating
2- blueray reading lens
Solution:
1- take your ps4 to repiere shop for cleaning or you can do it if have the right tools
2- you can buy lens clean kit and read the instructions how to use it
If non of these works try to restore the system to factory settings

I was having problem, exactly as described above. After much frustration it seems what has fixed the problem is simply downloading my online storage to the system. I'm not sure what exactly went wrong to begin with but so far so good. Looks like I'll have to do this with all of my games.

This happened to me too because I was installing dlc across two ps4 systems and taking them offline. You can run your games from both consoles but not offline. If the ps4 you're using is not your primary console it won't let you run the game without connecting to the internet to verify the game license.
