Same problem, headset works sometimes other times it doesn't. The problem seems to be the 3.5mm jack in conjunction with Microsoft's software and how it detects the headset when plugged in. If you put some pressure on one side of the cable where it plugs into the controller I could get it to detect the headset and work sometimes and sometimes only get sound out of one side of the headset. Definitely a Microsoft problem that needs to be addressed A.S.A.P. before another class action lawsuit.

You must make sure that the headset is plugged into the controller before you turn the Xbox One console on with the controller.
